It's now possible to download the inclusion forms and 14-day
follow-up form, please take care that the forms are printed in the right format, so before
printing read the following instructions:
- Make sure you use Acrobat Reader 5.0 or higher, if you have
an older version please upgrade

- When using Acrobat Reader use the following printing
options:
- print on A4-sized paper (you'll find
that under "print", "properties", "paper")
- use the option: expand small pages to
paper size (and not the other 2 options) (you'll find it under "print",
"copies and adjustments")
